Female coital orgasm may be an adaptation for preferentially retaining the sperm of males with “good genes.” One indicator of good genes may be physical attractiveness. Accordingly, R. Thornhill, S. W. Gangestad, and R. Comer (1995) found that women mated to more attractive men reported an orgasm during a greater proportion of copulations than did women mated to less attractive men. The current research replicates this finding, with several design variations. We collected self-report data from 388 women residing in the (...) United States or in Germany. Results support the hypothesis that women mated to more attractive men are more likely to report an orgasm at the most recent copulation than are women mated to less attractive men, after statistically controlling for several key variables. Discussion addresses (a) the inability of the present research to specify the causal link between female orgasm and male attractiveness and (b) the proactive nature of female sexuality documented in recent research guided by an evolutionary perspective. In this paper, we consider how the four key team emergent states for team learning identified by Bell, Kozlowski and Blawath (2012), namely psychological safety, goal orientation, cohesion, and efficacy, operate as a system that produces the team’s learning climate (TLC). Using the language of systems dynamics, we conceptualize TLC as a stock that rises and falls as a joint function of the psychological safety, goal orientation, cohesion, and efficacy that exists in the team. The systems approach highlights aspects of (...) TLC management that are traditionally overlooked, such as the simultaneous influence of and feedback between the four team emergent states and the inertia that TLC can have as a result. The management of TLC becomes an issue of controlling the system rather than each state as an independent force, especially because changing one part of the system will also affect other parts in sometimes unintended and undesirable ways. Thus the value is to offer a systems view on the leadership function of team monitoring with regards to team emergent states, which we term team state monitoring. This view offers promising avenues for future research as well as practical wisdom. It can help leaders remember that TLC represents an equilibrium that needs balance, in addition to pointing to the various ways in which they can influence such equilibrium. (shrink)

With armed conflict in the Persian Gulf now upon us, Harvard archaeologist Steven LeBlanc takes a long-term view of the nature and roots of war, presenting a controversial thesis: The notion of the "noble savage" living in peace with one another and in harmony with nature is a fantasy. In Constant Battles: The Myth of the Peaceful, Noble Savage , LeBlanc contends that warfare and violent conflict have existed throughout human history, and that humans have never lived in (...) ecological balance with nature. The start of the second major U.S. military action in the Persian Gulf, combined with regular headlines about spiraling environmental destruction, would tempt anyone to conclude that humankind is fast approaching a catastrophic end. But as LeBlanc brilliantly argues, the archaeological record shows that the warfare and ecological destruction we find today fit into patterns of human behavior that have gone on for millions of years. Constant Battles surveys human history in terms of social organization-from hunter gatherers, to tribal agriculturalists, to more complex societies. LeBlanc takes the reader on his own digs around the world -- from New Guinea to the Southwestern U.S. to Turkey -- to show how he has come to discover warfare everywhere at every time. His own fieldwork combined with his archaeological, ethnographic, and historical research, presents a riveting account of how, throughout human history, people always have outgrown the carrying capacity of their environment, which has led to war. Ultimately, though, LeBlanc's point of view is reassuring and optimistic. As he explains the roots of warfare in human history, he also demonstrates that warfare today has far less impact than it did in the past. He also argues that, as awareness of these patterns and the advantages of modern technology increase, so does our ability to avoid war in the future. (shrink)

Associer Leopold à Potter a pour effet de placer la bioéthique dans la famille des éthiques de l’environnement, ce qui la différencie du sens conventionnel retenu en médecine et en recherche depuis le Rapport Belmont (1979), une (...) déclaration ayant propulsé l’institutionnalisation de la bioéthique en Amérique du Nord. Cependant, diviser la bioéthique entre le médical et l’environnemental est réducteur. Potter propose au contraire une bioéthique globale s’intéressant aux enjeux situés à leur interface, dont ceux concernant la terre, la vie sauvage, la surpopulation, la consommation, etc. Cet article vise à amorcer un nouveau chantier d’analyse de la pensée de Potter en s’appuyant sur l’héritage de Leopold en biologie. Une synthèse de cette vision potterienne est proposée de manière à considérer son œuvre comme un tout cohérent s’intégrant aux grands débats qui transcendent les XXe et XXIe siècles. Sa vision apparaît comme une sagesse collective et prospective sous la forme d’une science de la survie et d’un code de bioéthique. In lieu of an abstract, here is a brief excerpt of the content:Memory and Justice:Narrative Sources of Community in Camus's The First ManJohn Randolph LeBlancThere as a certain frustration involved in trying to find Albert Camus's conception of justice in express positive statements. But inasmuch as Camus saw his work in the trope of journey, his complex set of ideas about justice are to be discerned in the narrative structure of his texts. This is particularly so in his last work, (...) the incomplete autobiographical The First Man.1 In the novel, Camus has his protagonist and fictional self Jacques Cormery seek the sources of his orientation to questions of love and justice by attempting to reconstruct the brief life of a father he never knew. It is in the life reconstructed, chiefly through the recollected memories of those who knew the man that Jacques comes to understand his own responses to questions of love and justice. A necessary by-product of the process is that through his questioning, Jacques recreates a community of which he is a product but, until the journey is undertaken, he has never been a part. His journey is, thus, one of self-discovery and inextricably ties justice to community formation. Camus's last novel, I will argue, suggests the importance of memory and narrative to the fundamental questions of political theory, particularly, questions regarding community-formation and the role and nature of justice.IContemporary reflections on the nature of politics take narrative encounters with reality seriously. The significance of narrative to political theory is, I think, tied to the Augustinian recognition that, temporally speaking, we live forward but learn backwards. It is through reconstructing [End Page 140] our experiences as individuals that we learn about who we are and how we got here. Recent political theory has come to recognize that this epistemological structure works at both the individual and the communal level. Michael Walzer, for example, has suggested that a community's culture is the story its members tell so as to make sense of the different pieces of their social life and justice is the doctrine that distinguishes the pieces.2 But distinguishing the pieces can mean and has meant blocking or restricting our access to that reconstruction. While a time-honored political practice, the impulse to control the contents of the narrative by deeming it unworthy or too dangerous to speak became a self-conscious political practice and a particularly dangerous phenomenon in the last century. Controlling the contents of narrative means control in past, present, and future tense. Memory can become a site of resistance because narrative depends on memory for its points of reference. What gets narrated gets remembered. Most often, what does not get narrated either gets lost or must find another way.Narrative and memory have always been political resources, Joshua Foa Dienstag suggests in his important work on memory and narrative in political theory, but the self-conscious control of them is dangerous.3 At the end of a century wherein we perpetrated much we might be tempted to forget, it is increasingly evident that we must remember before we can speak of justice, that is, our stories must be told and heard before we can reconstitute (or re-member) a functional conception of political community. It is in these terms that I want to talk about narrative and its relationship to political community and justice. But what is silenced is as important as what is articulated. Dienstag, who reads political theory as narrative, suggests that, practically speaking, what we have learned is that memory unarticulated is even more dangerous than memory manipulated. Memory unarticulated either festers and becomes a flashpoint for turmoil or is articulated by an interest for its own ends and becomes a tool of control if not oppression. In either case, memory is bastardized to the ends of power politics and distinguishing between authentic and inauthentic experiences becomes the province of politicians. Leaving these distinctions to politicians, I want to suggest, is risky business. We sought to identify emotional reactions to a partner's sexual infidelity and emotional infidelity. In a preliminary study, 53 participants nominated emotional reactions to a partner's sexual and emotional infidelity. In a second study, 655 participants rated each emotion for how likely it was to occur following sexual and emotional infidelity. Principal components analysis revealed 15 emotion components, including Hostile/Vengeful, Depressed, and Sexually aroused. We conducted repeated measures analyses of variance on the 15 components, with participant sex as the between-subjects (...) factor and infidelity type as the within-subjects factor. A main effect for sex obtained for 9 components. For example, men scored higher on Homicidal/Suicidal, whereas women scored higher on Undesirable/Insecure. A main effect for infidelity type obtained for 12 components. For example, participants endorsed Nauseated/Repulsed as more likely to follow sexual infidelity and Undesirable/Insecure as more likely to follow emotional infidelity. Discussion addresses limitations of this research, and highlights the need for an integrative theory of emotional reactions to infidelity. (shrink)

Gentzen's account of logical consequence is extended so as to become a matter of degree. We characterize and study two kinds of function G, where G(X,Y) takes values between 0 and 1, which represent the degree to which the set X of statements (understood conjunctively) logically implies the set Y of statements (understood disjunctively). It is then shown that these functions are essentially the same as the absolute and the relative probability functions described by Carnap.

On the basis of Anton Marty’s 1867 Preisschrift, this article offers a reconstruction of the semiotic and linguistic investigations the Swiss philosopher develops just before becoming a student of Brentano. The paper then compares this account with the view on signs that will be given in Marty’s later work, as well as within the Austro-German tradition.

For Van Rensselaer Potter (1911–2001), Global Bio-Ethics is about building on the legacy of Aldo Leopold (1887–1948), one of the most notable forest managers of the twentieth century who brought to light the importance of pragmatism in the sciences and showed us a new way to proceed with environmental ethics. Following Richard Huxtable and Jonathan Ives's methodological 'Framework for Empirical Bioethics Research Projects' called 'Mapping, framing, shaping,' published in BMC Medicine Ethics (2019)), we propose operationalizing a framework for Global Bio-Ethics (...) by hybridizing approaches in empirical bioethics and ecosystem management. We explain this framework using the metaphor of forest management. This mixed approach is articulated through three phases: (1) mapping the “landscape” to build a working theory, (2) framing the “scene” to prepare the fieldwork, and (3) shaping bioethics “tools” to stimulate cooperation. Applying this methodology, an adaptive management cycle is outlined to help ensure that political processes are sustainable and socially acceptable, still based on strategic and ethical thinking, but also capable of reshaping failing policies. Intellectual historians have rarely attended to the genre of fighting manuals, but these provide a new window on long-debated questions such as the relationship between Scholasticism and Humanism. This article offers a close comparison of the first known fencing manual, the 14-th century Liber de Arte Dimicatoria (Leeds, Royal Armouries FECHT 1, previously and better known as MS I.33), and the corpus of fighting manuals which underwent a remarkable expansion during the 15th and 16th centuries. While the former clearly shows (...) its origins in a scholastic background, the latter is mainly viewed as reflecting its humanist context. To this historiographical division corresponds a linguistic one: MS I.33 is a Latin text, while the rest of the corpus is mainly written in German and Italian. However, exceptions arise, amongst which, Heinrich von Gunterrodt’s Sciomachia et Hoplomachia: sive de Veris Principiis Artis Dimicatoriae (1579), the first text which explicitly refers to I.33. This article will compare these two texts, in order to interrogate their common relation to Scholasticism, namely the traditional frame of the knowledge within the medieval and early modern universities. The intent is to show that (at least some) Renaissance fight books include references to Scholasticism and to provide a better qualification of the nature of such references. The general hypothesis is that a large part of the texts―and products of culture―of the Renaissance that have been read, until recently, exclusively in relation to a humanist intellectual background can valuably be interpreted in the context of a Scholasticism that is still vivid during the period in question. Understanding arithmetic word problems involves a complex interaction of text comprehension and mathematical processes. This article presents a computer simulation designed to capture the working memory demands required in “bottomup” comprehension of arithmetic word problems. The simulation's sentence‐level parser and text integration component reflect the importance of processing the problem from its original natural language presentation. Children's probability of solution was analyzed in exploratory regression analyses as a function of the simulation's sentence‐level and text integration processes. Working memory variables measuring (...) the combined effects of concepts to remember and text integration inferences account for a significant proportion of variance in children's solution probabilities across the first four grade levels (K‐3). Consistent with previous results from others, which highlighted the significance of small changes in problem wording, the simulation offers a process‐oriented perspective as to why natural language presentation constrains the comprehension of mathematical relationships. Chemero's concept of sensorimotor empathy offers a relevant introduction to the study of human/non-human relationships. This article proposes an empirical characterization of this phenomenon occurring in human–horse interactions through the notion of 'contact', which is a core concept in the technical tradition of the equestrian world. According to the assumptions of 4E cognition, we approach the notion of contact with a broader meaning than how it is usually defined, i.e.as the connection of the rider's hand with the horse's mouth. We (...) state that contact involves the whole body and that it is not a passive touch but a dynamic one. Contact is intersubjective, meaning that its quality depends on the two agents of the interaction: the human and the horse. Within such an interaction, sensorimotor empathy allows the human and the horse to understand through the body and to fine-tune with each other to bring about 'good contact'. (shrink)

L’histoire de la pensée sémiotique se caractérise par une oscillation entre définition large et définition étroite de son objet. Au Moyen Âge, la définition augustinienne du signe est jugée trop étroite, car elle ne concerne que le signe sensible. De nouvelles définitions tentent alors de faire des concepts des signes qui renvoient aux choses. L’Âge moderne, au contraire, affirme une volonté de rétrécissement à l’égard de la notion de signe. Cet article montrera les caractéristiques d’une telle réflexion sémiotique à travers (...) deux auteurs représentatifs d’un premier empirisme moderne : Pierre Gassendi et Thomas Hobbes. Le retour à une sémiotique étroite s’effectue par la référence aux traditions stoïcienne, sceptique et épicurienne, ainsi qu’à un Aristote renouvelé, et par une prédilection affichée pour la valeur indiciaire du signe par rapport à sa valeur sémantique. Ceci veut dire que les empiristes vont insister avant tout sur la fonction remémorative du signe et sur sa valeur de probabilité. L’indice en tant que trace sera ainsi privilégié par rapport au signe communicationnel, et il s’agira d’en défendre la validité en tant qu’il s’inscrit dans une échelle de certitude, au sommet de laquelle se situe la preuve. Kolmogorov's account in his [1933] of an absolute probability space presupposes given a Boolean algebra, and so does Rényi's account in his [1955] and [1964] of a relative probability space. Anxious to prove probability theory ‘autonomous’. Popper supplied in his [1955] and [1957] accounts of probability spaces of which Boolean algebras are not and [1957] accounts of probability spaces of which fields are not prerequisites but byproducts instead.1 I review the accounts in question, showing how Popper's issue from and how (...) they differ from Kolmogorov's and Rényi's, and I examine on closing Popper's notion of ‘autonomous independence’. So as not to interrupt the exposition, I allow myself in the main text but a few proofs, relegating others to the Appendix and indicating as I go along where in the literature the rest can be found. The most disturbing gift that Disgrace presents to its readers is the hushed resolve with which Lucy Lurie emerges from her rape to reaffirm her way of life. To consider that way of life, the reader is first invited to align oneself with David Lurie's initial normative reading of his daughter's rape; but then, in a second important step, to join in the change of mind by which David overcomes this initial blindness. Imagine what accepting the invitation to take both (...) of these steps demands of the reader: Will you let yourself undergo a change of mind with David to resee Lucy as resolute and proactive? Shown here is that a constraint used by Popper in The Logic of Scientific Discovery (1959) for calculating the absolute probability of a universal quantification, and one introduced by Stalnaker in "Probability and Conditionals" (1970, 70) for calculating the relative probability of a negation, are too weak for the job. The constraint wanted in the first case is in Bendall (1979) and that wanted in the second case is in Popper (1959).

This paper discusses the issue of overriding the right of individual consent to participation in cluster randomised trials (CRTs). We focus on CRTs testing the efficacy of non-pharmaceutical interventions. As an example, we consider school closures during the COVID-19 pandemic. In Norway, a CRT was promoted as necessary for providing the best evidence to inform pandemic management policy. However, the proposal was rejected by the Norwegian Research Ethics Committee since it would violate the requirement for individual informed consent. This sparked (...) debate about whether ethics stand in the way of evidence-based health policy, since the Norwegian Research Ethics law’s strict requirements for individual consent make it practically impossible to carry out CRTs of public health interventions. We argue that, in the case of the school closure trial, the suggested CRT would not have eliminated an epistemic gap and thus would not have justified the violation of consent rights. First, we focus on the methodological challenges to estimating quantifiable effects of school closures in the specific case of an airborne infectious disease. Second, in line with Evidential Pluralism, we highlight the value of alternative lines of evidence for informing school closure policy in a pandemic. In general, we propose that a trial requiring the waiver of participants’ consent rights must be highly likely to eliminate an epistemic gap. We elaborate on the practical aspects of this criterion and discuss the potential advantages of adding it to the Ottawa Statement on the Ethical Design and Conduct of Cluster Randomized Trials. La réception française d’Heinrich Rickert (1863-1936), au début du xxe siècle, s’inscrit tout d’abord au cœur des débats épistémologiques autour de la constitution des sciences historiques et des sciences sociales, ainsi que l’atteste la controverse entre Paul Lacombe et Alexandru Dimitri Xenopol. Le second volet de la réception de Rickert en France concerne la philosophie des valeurs et le lien qu’elle entretient avec les positions épistémologiques défendues par le néokantien de Bade. L’étude de cet aspect de la réception se fera (...) à partir de la lecture qu’en proposent Georges Gurvitch d’une part, et Raymond Aron d’autre part, tous deux formulant, à leur manière, une accusation visant le formalisme de la théorie rickertienne des valeurs. Si les discussions philosophiques des positions du philosophe allemand sont assez rares, elles constituent néanmoins un révélateur pertinent des débats qui animaient la philosophie et les sciences historiques et sociales de l’époque. The debate between Husserl and Marty focuses on the notion of general grammar. Nevertheless, there doesn’t seem to have been a clear outcome, and the terms of the debate remain quite unclear. Moreover, while both authors make striking use of historical references, their entanglement seems to call for some clarification. This paper aims to shed light on this debate, by considering it from an historical perspective. In doing so, two putative candidates will be introduced as (conceptual) precursors of the ‘allgemeine (...) Grammatik’: the Grammaire générale et raisonnée of Port-Royal (1660), and the medieval grammatica speculativa, which included a group of late thirteenth- and early fourteenth-century philosophers of language known as the Modistae. I will first identify Husserl’s and Marty’s respective critiques, distinguishing, between Husserl’s critique of the conflation of the a priori and empirical, and Marty’s of the conflation of meaning and forms of expression. Second, I will place these critiques in an arrangement involving two types of cross-reference: on the one hand, Marty and Husserl can be seen as standing in opposition to their historical predecessors; on the other hand (and more significantly), Marty and Port-Royal can be seen as standing in opposition to Husserl and the speculative grammarians. Parmi les différentes approches possibles de la matière historique, on observe souvent, dans la littérature, une tension entre les deux options suivantes : faire d’un auteur le précurseur d'une révolution dont notre modernité serait l'héritière directe, ou au contraire, et par réaction, se livrer à un travail de remise en contexte détaillé qui prend parfois le risque de gommer l'originalité possible de ce même auteur. Le Traité sur les signes de Jean Poinsot, dominicain du début du XVIIe siècle, a ainsi (...) d’abord été édité comme un texte annonciateur de la sémiotique de Peirce, la thèse de cette originalité révolutionnaire étant fondée sur le traitement de la catégorie de la relation. Le travail critique qui s’est effectué en réaction tempère, parfois de façon excessive, la nouveauté de cet auteur du point de vue de sa contribution à la pensée des signes. Cet article s’inscrit dans cette démarche de remise en contexte en proposant une comparaison entre le texte de Poinsot et les Quaestiones sur les signes, que l'on doit à Sebastião do Couto, commentateur de l'université de Coimbra. Optant pour une contextualisation qui vise à mesurer l’originalité relative de Poinsot, cette étude porte sur les variations de la description du rapport du signe à l'intention. Parmi les différentes approches possibles de la matière historique, on observe souvent, dans la littérature, une tension entre les deux options suivantes : faire d’un auteur le précurseur d'une révolution dont notre modernité serait l'héritière directe, ou au contraire, et par réaction, se livrer à un travail de remise en contexte détaillé qui prend parfois le risque de gommer l'originalité possible de ce même auteur. Le Traité sur les signes de Jean Poinsot (appelé également Jean de Saint Thomas), dominicain du (...) début du XVIIe siècle, a ainsi d’abord été édité comme un texte annonciateur de la sémiotique de Peirce, la thèse de cette originalité révolutionnaire étant fondée sur le traitement de la catégorie de la relation. Le travail critique qui s’est effectué en réaction tempère, parfois de façon excessive, la nouveauté de cet auteur du point de vue de sa contribution à la pensée des signes. Cet article s’inscrit dans cette démarche de remise en contexte en proposant une comparaison entre le texte de Poinsot et les Quaestiones sur les signes, que l'on doit à Sebastião do Couto, commentateur de l'université de Coimbra. Optant pour une contextualisation qui vise à mesurer l’originalité relative de Poinsot, cette étude porte sur les variations de la description du rapport du signe à l'intention. Le milieu universitaire, précisément le monde des enseignants-chercheurs, se trouve dans la nécessité absolue de s’approprier les nouvelles technologies de l’information ; en même temps, il est l’héritier d’habitudes épistémologiques séculaires. Cet article pose la question des pratiques présentes et émergentes dans les processus de documentation scientifique. Il s’agit d’une part de constater les éventuelles carences dénoncées par les chercheurs en matière d’accès à l’information, d’autre part de faire émerger tout un système de représentations sociales participant d’une culture informationnelle spécifique (...) influençant nécessairement les pratiques. Sur la base d’une analyse qualitative, nous mettons en évidence la nécessité, pour les chercheurs, quelle que soit leur discipline, de maintenir une relation « papier » avec les supports d’information. Cette relation traditionnelle permet de mieux appréhender l’innovation technique et de faire face aux potentialités offertes par les nouvelles sources d’accès à l’information. En outre, nous montrons que la définition même du métier de chercheur et son comportement face au traitement de l’information dépend de sa culture disciplinaire, selon qu’il appartient aux sciences humaines et sociales ou aux « sciences dures ».The university environment, where scholars and professors work, is faced with an imperative need to appropriate the new information technologies, but it has also inherited the epistemological habits of centuries. This article raises the question of current and emerging practices in the scientific documentation process. It seeks on the one hand to observe the failings exposed by researchers regarding access to information, and on the other hand to draw out the contours of a whole system of social representations relating to a specific information culture that necessarily influences practice. Based on a qualitative analysis, we show the need for researchers in any discipline to maintain « print » relationships with information media. This traditional relationship helps to gain a better grasp of technical innovation and to cope with the potential of new ways of accessing information. We also show that the very definition of the research profession, as well as researchers’ attitudes to information processing, depend on their disciplinary culture and whether they belong to the humanities and social sciences or to the « hard sciences ». Provided here is a characterisation of absolute probability functions for intuitionistic (propositional) logic L, i.e. a set of constraints on the unary functions P from the statements of L to the reals, which insures that (i) if a statement A of L is provable in L, then P(A) = 1 for every P, L's axiomatisation being thus sound in the probabilistic sense, and (ii) if P(A) = 1 for every P, then A is provable in L, L's axiomatisation being thus (...) complete in the probabilistic sense. As there are theorems of classical (propositional) logic that are not intuitionistic ones, there are unary probability functions for intuitionistic logic that are not classical ones. At Symposium No. 14, which took place as part of the 7th International Symposium on Education, a round table discussion was held with five speakers from the field of education at different levels of teaching. This round table was an opportunity to discuss the issues associated with the link between beliefs and pedagogical practices among teachers at different levels of teaching, as identified by the various papers, and to review experiences or training systems implemented to help support teachers in this (...) regard. The participants shared their experience of accompaniment future and beginning teachers, highlighting the discomforts felt by future and beginning teachers that corresponds to elements often raised in the literature. There were also processes put in place, depending on the different situations, to promote professional integration and integration into a school team, both through co-development processes and group or individual reflections on the knowledge and conceptions of these teachers who were coached. The analysis of their professional practice is a focal point reported by all the participants. (shrink)

Many elementary school students have difficulties in reading and writing. However, researches showed that effective teaching practices have positive impacts in the success of students at school precisely in their reading and writing learning process. This paper presents results of a study on reported reading and writing teaching practices conducted on four sixth grade teachers by following a literature review on social constructivist theories winning practices. The results showed that they use innovative practices that incorporate reading, writing and oral communication (...) teachings in class ; even if they still have traditional practices such as dictation. They also focused on interdisciplinary and cross-functional projects. Those activities are more significant because of their closeness with the children’s reality and interests. In sum, the interviews revealed the teachers’ methods as a whole and an open-plan, which many effective practices studies recommend. (shrink)

S. A. Lloyd proposes a radically new interpretation of Hobbes's Leviathan that shows transcendent interests - interests that override the fear of death - to be crucial to both Hobbes's analysis of social disorder and his proposed remedy to it. Most previous commentators in the analytic philosophical tradition have argued that Hobbes thought that credible threats of physical force could be sufficient to deter people from political insurrection. Professor Lloyd convincingly shows that because Hobbes took the transcendence of religious and (...) moral interests seriously, he never believed that mere physical force could ensure social order. Lloyd's interpretation demonstrates the ineliminability of that half of Leviathan devoted to religion, and attributes to Hobbes a much more plausible conception of human nature than the narrow psychological egoism traditionally attributed to Hobbes. Next SectionBackground: Physicians face ethical difficulties daily, yet they seek ethics consultation infrequently. To date, no systematic data have been collected on the strategies they use to resolve such difficulties when they do so without the help of ethics consultation. Thus, our understanding of ethical decision making in day to day medical practice is poor. We report findings from the qualitative analysis of 310 ethically difficult situations described to us by physicians who encountered them in their practice. When facing such (...) situations, the physicians sought to avoid conflict, obtain assistance, and protect the integrity of their conscience and reputation, as well as the integrity of the group of people who participated in the decisions. These goals could conflict with each other, or with ethical goals, in problematic ways. Being aware of these potentially conflicting goals may help physicians to resolve ethical difficulties more effectively. This awareness should also contribute to informing the practice of ethics consultation. Objective: To identify strategies used by physicians in dealing with ethical difficulties in their practice. Design, setting, and participants: National survey of internists, oncologists, and intensive care specialists by computer assisted telephone interviews (n = 344, response rate = 64%). As part of this survey, we asked physicians to tell us about a recent ethical dilemma they had encountered in their medical practice. Transcripts of their open-ended responses were analysed using coding and analytical elements of the grounded theory approach. Main measurements: Strategies and approaches reported by respondents as part of their account of a recent ethical difficulty they had encountered in their practice. Results: When faced with ethical difficulties, the physicians avoided conflict and looked for assistance, which contributed to protecting, or attempting to protect, the integrity of their conscience and reputation, as well as the integrity of the group of people who participated in the decisions. These efforts sometimes reinforced ethical goals, such as following patients’ wishes or their best interests, but they sometimes competed with them. The goals of avoiding conflict, obtaining assistance, and protecting the respondent’s integrity and that of the group of decision makers could also compete with each other. Conclusion: In resolving ethical difficulties in medical practice, internists entertained competing goals that they did not always successfully achieve. Additionally, the means employed were not always the most likely to achieve those aims. Understanding these aspects of ethical decision making in medical practice is important both for physicians themselves as they struggle with ethical difficulties and for the ethics consultants who wish to help them in this process. Background: Ethics support services are growing in Europe to help doctors in dealing with ethical difficulties. Currently, insufficient attention has been focused on the experiences of doctors who have faced ethical difficulties in these countries to provide an evidence base for the development of these services.Methods: A survey instrument was adapted to explore the types of ethical dilemma faced by European doctors, how they ranked the difficulty of these dilemmas, their satisfaction with the resolution of a recent ethically difficult case (...) and the types of help they would consider useful. The questionnaire was translated and given to general internists in Norway, Switzerland, Italy and the UK.Results: Survey respondents ranged in age from 28 to 82 years, and averaged 25 years in practice. Only a minority reported having access to ethics consultation in individual cases. The ethical difficulties most often reported as being encountered were uncertain or impaired decision-making capacity , disagreement among caregivers and limitation of treatment at the end of life . The frequency of most ethical difficulties varied among countries, as did the type of issue considered most difficult. The types of help most often identified as potentially useful were professional reassurance about the decision being correct , someone capable of providing specific advice , help in weighing outcomes and clarification of the issues . Few of the types of help expected to be useful varied among countries.Conclusion: Cultural differences may indeed influence how doctors perceive ethical difficulties. The type of help needed, however, did not vary markedly. The general structure of ethics support services would not have to be radically altered to suit cultural variations among the surveyed countries. (shrink)

As support for clinical ethics committees in the UK grows, care must be taken to define their function, membership and method of working and the status of their decisions.The modern practice of medicine raises a plethora of complex issues—medical, ethical and legal. Doctors and other healthcare professionals increasingly must try to resolve these and may sometimes have to do so in the face of contrary opinion expressed by patients and/or their surrogates. While clearly qualified in the medical arena, and although (...) there is now more ethics teaching in the medical curriculum, healthcare professionals are seldom qualified to adjudicate on ethical or legal matters, or even, perhaps, to recognise them when they arise. Yet, as Doyal says, “clinical life must go on and moral and legal indeterminacy within medicine cries out for practical resolution.”1 Meanwhile, the expectations of patients and their families—and, indeed, of wider society—are that decisions about patient care, resources and therapeutic regimes should be soundly based on appropriate ethicolegal, as well as scientific, principle.Recognition of these additional burdens on healthcare professionals has generated some interest in the provision of ethics consultation. In the USA this has been undertaken by a variety of tribunals and individuals. Hospital ethics committees and ethics consultants feature in many US hospitals and perform a variety of roles. In Europe, a number of countries have also established clinical ethics committees and they are becoming more common in the UK—primarily in England and Wales for the moment.
